Why Graft Placement Matters in Hair Restoration Surgery

In hair restoration surgery, much attention is paid to donor harvesting, graft counts, and hairline design. Those elements matter—but there is one step that ultimately determines whether a transplant looks natural or fails outright: the placement of follicular unit grafts. This is not a minor technical detail. It is one of the most critical components of the entire procedure.

The Technical Precision Required for Follicular Unit Graft Placement

Placing follicular unit grafts is a meticulous, technically demanding process. Each graft must be inserted at the correct angle, direction, and depth to replicate natural hair growth. Even small errors can result in unnatural direction, poor density, or an obvious “transplant” appearance. Once grafts are placed incorrectly, the result cannot be easily corrected. In addition, follicular units are living tissue and must be handled gently during placement—poor technique or excessive manipulation can compromise graft survival.

This is precisely why graft placement is not a step I consider minor or delegable.

At the Gabel Hair Restoration Center, I personally place a majority of the follicular unit grafts in every procedure. I have over 20 years of hands-on experience placing grafts, and I am ultimately responsible for the outcome of each case. That responsibility does not end with making the recipient sites—it extends through the final graft placed. I am involved from start to finish.

In addition, the assistant working directly with me has nearly a decade of experience placing follicular unit grafts. My patients know exactly who is placing their grafts and what level of experience each person brings to the procedure. That level of transparency is uncommon in this field.

In many hair restoration clinics, graft placement is performed entirely by technicians, with little or no physician involvement. Patients are often unaware of who is actually placing their grafts, how long that individual has been doing so, or what level of training and oversight is involved. Because the physician does not participate in graft placement, multiple staff members may rotate through this role—sometimes with widely varying experience and skill levels—without the patient ever being informed.

Some clinics view graft placement as a routine or interchangeable task. I do not. Too much of the success of a hair transplant rests on this step to treat it casually. A perfectly designed hairline means nothing if the grafts are placed at the wrong angle. Excellent harvesting means little if graft survival is compromised during placement.

A Question Every Hair Restoration Patient Should Ask

For anyone considering hair restoration, I believe there is a critical question that should always be asked:

“Who will be placing the grafts for my case?”

If the physician is not placing the grafts, you should ask why. When the experience level of the individuals placing grafts is unclear or undisclosed, that should raise concern.
